Please Note: While there are no scheduled weekend or holiday shifts, the Nurse must be available and responsive by phone or text after hours to assist with urgent medical concerns and emergencies as needed.
Position Summary
The Nurse is responsible for coordinating and monitoring healthcare services for individuals served by Wabash Center. This role works closely with staff, families, guardians, medical providers, and interdisciplinary team members to ensure quality healthcare, regulatory compliance, and positive health outcomes.
The Nurse also provides medication administration training and ongoing healthcare education to staff in residential and day services.

Essential Responsibilities
- Train staff in medication administration using the Indiana state curriculum
- Monitor medication administration practices
- Complete Medicaid Waiver wellness assessments and required face-to-face visits
- Maintain medical records and physician orders
- Coordinate medical appointments and required follow-up care
- Assess healthcare needs and recommend appropriate services
- Complete quarterly wellness reports
- Provide employee TB testing
- Ensure compliance with Medicaid Waiver regulations
- Develop and update health-related risk plans
- Educate individuals served on health and wellness topics
- Participate in interdisciplinary team meetings
- Present staff in-service trainings
- Respond to urgent medical issues after business hours as needed
Qualifications
- Current Indiana Nursing License required
- Strong assessment and healthcare coordination skills
- Ability to maintain accurate medical documentation
- Excellent communication and relationship-building skills
- Ability to work independently and exercise sound judgment
- Valid driver's license, reliable transportation, proof of insurance, and acceptable driving record
- Ability to complete required training and certifications

Salary: $63,000-$65,000 per year
